COVID-19 Vaccine for Ouray County Residents

If you live in San Miguel County, please read this blog post for information regarding how to sign up to be notified when the vaccine is available for your phase.

For Mountain Medical Center patients who live in Ouray County:

Good news! The Ouray County Public Health Agency (OCPHA) has begun receiving the two-part Moderna COVID-19 vaccine. There are a limited number of vaccines available at this time, so please check out the graphic below to determine if you qualify for the current phase.

For questions regarding the COVID-19 vaccine, please check out their website and email the county at if you need further clarification. MMC is not the distributor for this vaccine and will not be able to answer questions regarding availability and phasing.

One you have determined your phase, please click on this link to submit your contact information and answer a few questions for the OCPHA. Please note that you currently have to request access to the form, so it may take a bit to receive access.
